This is a mate in five moves from a game which I just lost.  I resigned before the end, but see if you can find the end in five moves like my opponent would have.

I hope to include more puzzles that come from my actual games whenever I find one.  Hopefully they will be unique and have only one solution.  Feel free to leave comments.
