Looking forward to the notes.
I didn't like White's Bh3 and Bg4 (2 moves) and then not playing Bxh5 immediately, presumably too embarrassed to make 3 Bishop moves so quickly! White play of Bg2, Nf3 and O-O seemed more prudent.
Once he hadn't played Bxh5, your f5 gave White a lot of trouble OTB. He felt forced to play Bxh5 (which he should already have done) and I think it was a big mistake, although tempting, for him to play exf5 opening up your Bg7 and your Q on the same file as his K. After that it was really easy for his game to fall apart, which it did very quickly.
I wasn't sure about Qe8 objectively but maybe you were into the mind-set of your opponent and figured it would scare him off Bxh5 immediately or tempt him into over-aggressive pawn swaps. Certainly worked :-)

Three thoughts:
1) Congratulations on a fine game.
2) I have just started playing the Czech Benoni, so good timing.
3) What was your thinking behind 11...Qe8 ? As White I wouldn't have been able to resist 12.Bxh5 followed by Qd2, Bh6 and infiltrating with the Queen to g5.

15.Nxe4 is the sharpest continuation, but your opponent was obviously a bit of a wimp.

15. Qxe4 is the weakest way to capture, but it is the most likely to be played by a human in my opinion because it offers a queen exchange (2 pawns up remember!), ...Ne5 doesn't come with tempo and also the g4 square isn't weakened.
15.fxe4 is probably best and white should have a small advantage, but black still has the easier game to play in practice which is hugely important!

15.Qxe4 seems like a weak move to me. Guess he was expecting QxQ, pxQ. Is whites position move solid if 15.cxd is played?
