yes but it does it even when i fail a problem then click exit

that can happen when you close the window to a puzzle before you've finished it. the tactics trainer will count it as a failed puzzle, and you can see why, as someone could exploit that (if it didn't penalize for closing the window prematurely) if they came across a puzzle they couldn't solve.
